Your Vasectomy
Although vasectomy techniques vary, all vasectomy procedures involve causing blockage in the vas deferens in a very similar manner. As a result, the vast majority of vasectomies can be surgically reversed.
If your vasectomy was performed more than 12 years ago, a successful reversal is still possible. However, the likelihood of recovering sperm and achieving a pregnancy may be lower than it is for men who had a more recent vasectomy.
Your Health
Most men considering a vasectomy reversal are healthy candidates for outpatient surgery. In general, most medical conditions do not prevent you from safely undergoing the procedure.
However, certain serious medical conditions may increase the risks associated with outpatient surgery. These include significant heart disease (such as heart failure, a prior heart attack, or an implanted pacemaker), severe lung disease that affects oxygen levels, advanced liver disease, or significant kidney disease, including kidney failure or the need for dialysis.

What are the risks of vasectomy reversal?
Like any surgical procedure, vasectomy reversal has risks. The two most important risks are:
Scrotal hematoma (bleeding into the scrotum)
Failure to restore fertility
Scrotal Hematoma
A scrotal hematoma is caused by bleeding within the scrotum during the first 24 hours after surgery. The blood collects and forms a clot, known medically as a hematoma.
The risk of developing a scrotal hematoma after vasectomy reversal is less than 5%. Most hematomas are diagnosed within the first 24 hours after surgery.
In most cases, no additional treatment is required. The body gradually breaks down and absorbs the blood clot over the course of several weeks. During this time, patients may experience increased swelling, bruising, and discomfort before the hematoma slowly resolves.
Failure to Restore Fertility
The greatest risk of vasectomy reversal is that the procedure may not restore fertility. For this reason, a vasectomy should always be considered a permanent form of contraception, as no surgeon can guarantee a successful reversal.
There are several reasons why fertility may not return after surgery:
The repaired vas deferens may heal closed after the reversal, preventing sperm from reaching the semen.
Years of blockage after a vasectomy may cause damage to the epididymis, reducing or preventing the passage of sperm.
Even when sperm return to the semen, pregnancy is not guaranteed. Once all of your questions have been answered and you are ready to schedule, the process is simple:
- Complete our online health questionnaire and pay the scheduling fee at the end of the questionnaire.
- Complete a telephone health interview with a member of our nursing staff. During this call, you will select your surgery date, and we will email your scheduling documents.
- Electronically sign and return all required documents.
- Pay your surgical fee in full within 24 hours of completing your telephone health interview.
- Travel to Raleigh, NC, the evening before your scheduled surgery.
- Meet with your surgeon for your preoperative consultation, then check into your hotel.
- Undergo your vasectomy reversal the following morning and return to your hotel after surgery.
- Return to our office the morning after surgery for your postoperative follow-up visit, then travel home.

Do I need a driver?
Because vasectomy reversal is performed under general anesthesia, you must have a responsible adult (18 years of age or older) accompany you to your preoperative consultation and on the day of your surgery.
If you arrive for surgery without a responsible adult, your procedure will be canceled, and your surgical fee will be forfeited.
If it is not possible for a responsible adult to accompany you to your preoperative consultation, we may make exceptions on a case-by-case basis. However, a responsible adult is required on the day of your surgery.
We also recommend that your responsible adult accompany you to your postoperative follow-up visit and assist you with your travel home afterward.
How long is the recovery?
The Day of Surgery
You will likely be very sleepy for the first 3 to 8 hours after surgery because of the effects of general anesthesia. We recommend returning to your hotel and resting for the remainder of the day. Most patients report very little discomfort on the evening of surgery.
The First 24 Hours
By the day after surgery, most patients feel remarkably well. You should be able to walk, eat, and move around normally. Most patients rate their discomfort as approximately 2 out of 10. Ice pack and frozen peas will not be required. Any discomfort is usually well controlled with over-the-counter anti-inflammatory medications, such as ibuprofen, unless otherwise instructed by your surgeon.
The First 48 Hours
During the first 48 hours, you should be able to walk, sit, lie down, and sleep comfortably. Ice pack and frozen peas will not be required. Most patients continue to experience only minimal discomfort.
We recommend wearing the surgical scrotal support continuously during this time. After 48 hours, you may remove the support and take your first shower.
We recommend showering with a easily available over the counter surgical scrub soap and the nurse will discuss this with you during scheduling.
Weeks 1–2
After your first shower, we recommend wearing the athletic scrotal supporter that we provide for an additional two weeks. This extra support helps improve comfort during your recovery.
Please avoid sexual activity during the first two weeks after surgery.
Returning to Work
If you have a desk job or other sedentary work, you may typically return to work within 48 hours.
If your job involves manual labor, heavy lifting, or frequent bending, we recommend avoiding these activities for the first two weeks after surgery.
After Two Weeks
If you are no longer experiencing significant discomfort, you may discontinue the athletic supporter and resume sexual activity.
If you anticipate strenuous physical activity or heavy lifting, we recommend continuing to wear the athletic supporter for additional comfort and support.
Weeks 2–4
Although most patients feel well after two weeks, we recommend avoiding unnecessary heavy lifting and strenuous exercise, such as weightlifting at the gym, until four weeks after surgery whenever possible.
Semen Sample Testing
We offer a convenient home-based semen analysis program. You will order the test directly from our laboratory partner, which will mail the collection kit to your home. The sample can be collected in the privacy of your home and returned to the laboratory by mail. Patients are responsible for the cost of this test.
We recommend your first semen analysis approximately 3 months after vasectomy reversal surgery. If no sperm are seen, we recommend repeating the test at 6 months after surgery.
Once sperm have returned, we recommend attempting pregnancy for one year. If pregnancy has not occurred after one year, we recommend repeating the semen analysis to evaluate your sperm count and discuss any additional recommendations.
